Team 1: Reducing the time from triage to seen by a physician or reassessment, Victoria Hospital Emergency

Key accomplishments:

  • Reduced the number of patients leaving the ED without being seen by 88%
  • Reduced traige nurse walking distance by 82%
  • Reduced lead time from triage to being seen by physician/ reassessment by 80%

Team 2: Reduce the number of errors when reporting laboratory results, Victoria Hospital Laboratory

Key accomplishments:

  • Achieved zero defects in reporting laboratory results
  • Reduced interruptions for technician by 97.5%
  • Reduced rework by 27.8%
  • Reduced lead time for results release by 94.6%

Team 3: Reduce the risk of medication errors in the Victoria Hospital OR

Key accomplishments:

  • Established standard work and process for labelling medications (error rate reduced to 0)
  • Established standard work for restocking of anesthetic medication cart
  • Reduced cycle time with simpler labelling process for selecting, drawing and labelling medications.
